I really love this CD. It completes my Nelson collection. I own all 8 albums. The only part that I didn't care for was the fact that all tracks come from their first two albums. Matthew and Gunnar have several excellent songs from their other albums. With the majority of their other albums being released in the late 90's it would have been nice to of had a little more variety on the millennium album.

If you liked the Nelson brothers in the early 90's (when they first appeared), you will love this hit-packed CD. Most of their songs have a timeless resonance ranging from pop to rock. A couple of slower melodies and their words are nice to kick back and bask in, as their meanings eventually play out in your own head. Excellent choice for those who appreciate a well thought out attempt by two extraordinary musicians and their talented band members.

I know it's easy to write off this band as a cheesy 90's relic or novelty act, but if you can look past the spandex outfits and all that hair, you would see that Nelson released some seriously catchy, high quality melodic rock. Nelson's powerful melodic sound was the logical follow up to the Journey/Foreigner/Loverboy arena rock sound of the 80's. People seem quick to trash that kind of music now, but all of those bands - Nelson included - sold tons of albums back in the day, and you know darn well that when you're caught singing along to the radio in your car, it's going to be one of those band's songs you're belting out!



Say what you will about their image, but the well-crafted, radio-friendly rock songs on Nelson's 1990 debut After the Rain are everything good pop/rock is supposed to be. There are tons of great vocal harmonies, guitar hooks for miles, and all of the energy of a musical scene that was at its highest peak. Sure the end was near, but you wouldn't know it listening to this album. The album's highlight is the title track (which is still one of my favorite songs), but there are plenty of great songs to choose from, including the single (Can't Live without Your) Love and Affection. Even the ballad Only Time Will Tell is a fantastic song.



My only complaint is that After the Rain still hasn't been reissued with digitally remastered sound. The 20th Century Masters: Best of Nelson disc featured higher quality versions of about half of the songs on After the Rain, but I'd love to hear the whole thing remastered.



Laugh if you must, but I am not the least bit ashamed to sing the praises of this album. After the Rain is a first rate melodic rock album, and is perhaps the best example of what the genre had to offer in the late 80's and early 90's.

This was the only successful album by Nelson, a pair of brothers Matt and Gunner Nelson. Contains a number of hit singles including Love and Affection. To make a career in music, you need to be an 80s hair band or be forced to slumping sales and retirement. It's somewhere between hard rock and country. I like it.

Back in '90 when this album came out, I was a 12 year old fan who played my Nelson cassette until I wore it out. (Thankfully I had made extra copies..)I don't know how much I knew at 12 but I knew that I LOVED that tape, those guys, and the songs. After 10 years of waiting I finally got to see Nelson live in concert a few weeks ago (it was amazing!) and finally purchased After The Rain on CD. These songs are catchy, upbeat, and fun. They remind me of how simple yet fun it was to be 12 years old and the best part is they are ageless. I can still play it as loud as I did back then and it never dawns on me how old these songs are. My personal favorite track is "More Than Ever" because it never fails to make me smile or sing it at the top of my lungs. I was a fan at 12, I'm a fan at 22, and I will be a fan for as long as these guys make music because they are just awesome. If you don't already have it, GET THIS CD!! You won't regret it.

Ok everyone raves about this book. It is true that this book covers everything you need to know. However, the trick is not to get lost in their ridiculously long explanations that are repeated over and over and over each chapter. Seriously, this book could be condensed to half the size. Further, they mention a couple sentences about a concept and then quit and say "more on this to come later". They probably do that about a thousand times. Truthfully, i just got annoyed by reading the book. It is obnoxious and everything, from their graphs to their flow charts, is made to sound more complicated that it has to be and is encoded in erroneous/needless fluff. Medical students do not have much time, THAT IS TO SAY time is valuable, WHICH MEANS there is not much of it, SO YOU COULD CONCLUDE you should use time efficiently AND THIS MEANS... - if you found the previous sentence pointless and annoying, go with something else, perhaps BRS!

"I ordered "The Textbook of Medical Physiology" 11th edition by Guyton and Hall. The books (a 2 volume set) came but the covers look like they have water damage and the pages look photocopied. The text was supposed to have colored pictures, but they are all black and white. The binding is broken on both of the books and when you open the books the pages are not firmly attached and are falling out in clumps due to binding problems. The covers are not firmly attached to the binding of the book either. Overall, these appear to be copies of the text, and nothing similar to the actual book. It's almost as if it was photocopied and put in a binding. The print doesn't even line up properly on the page The figures are only partially visible, and there is noticeable outlining (like from photo copying). Having seen several copies of the book, this is definitely NOT the original text. ". I sent a few emails which were answered and they refused to refund my money claiming I could have already read the 1000 pages of the text book. Over all I WILL NOT AND WOULD NOT buy anything from this seller.

This is by far the best book out there for anyone thinking of writing a textbook. Lepionka is an experienced development editor, and it shows. As a textbook author, I can say that nearly everything she says rings true. She provides a detailed step-by-step account of how to attract the attention of a publisher, how to prepare a prospectus, what to expect during contract negotiations, how to interact with a development editor, and so on. This is information that is difficult to find elsewhere.



There are two points that I would like to add to her discussion. First, in signing a contract give very careful consideration to royalties on electronic versions of your text. The publishers are making a strong push to convert instructors and student to using electronic texts. Boilerplate contracts typically specify very low royalties on these sales. Even a couple of years ago, this didn't much matter because electronic texts had a very limited market. These days, though, you could lose a good chunk of your royalties if you don't negotiate a decent rate on electronic copies. Second, there is no longer much money to be made in writing texts for upper division or graduate courses. The used book market is so well organized, that a good part of your sales will be taken by resold comp copies. It is not at all unusual these days for even moderately successful upper division books to earn just a few thousand dollars in royalties -- less than you could probably earn teaching a summer course or two.



There a couple of essays on textbook writing available on the web. Be cautious in accepting their discussions of royalties. The one written by an accounting author (whose own text only lasted one edition) gives a very inflated idea of how much you are likely to earn in royalties.



Finally, even though I already owned the first edition, I couldn't resist buying this second edition. I was not disappointed. She did an excellent job or revising and updating the discussion.

Very inaccurate on ME (CFS). For example, it says ME is a functional somatic syndrome caused by depression and anxiety citing as proof that there is more psychiatric comorbidity in ME patients than healthy controls (p.638). This ignores, inter alia, the fact that the rates are no higher than for other disabling somatic diseases like MS or AIDS. It also recommends pushing the child back toward regular functioning which sometimes greatly increases morbidity. There are over 4,000 articles in peer reviewed medical journals showing frank biological pathology in ME. ME causes an enormous amount of morbidity and suffering. The propagation of falsehoods like these contribute to a huge amount of iatrogenic morbidity (and sometimes mortality) for people with ME.

This is the first textbook aimed at pediatricians who practice predominantly or exclusively in the hospital setting. Hospital medicine is more similar to emergency medicine as opposed to outpatient practice. Therefore, established pediatrics textbooks such as Nelson, Oski and Rudolph are less practical for hospital medicine. Those books cannot focus on inpatient conditions with sufficient depth and still remain relatively small (ie 1 volume or 2 volumes) and inexpensive. This hospital medicine book is a nice attempt to cover the scope of hospital practice. However, the quality of the chapters is variable. Some are excellent while others lack practical management strategies. Residents will find this book useful as it covers the scope of hospital practice more effectively than the established pediatric textbooks.

useless. except for bookends...I used to have this on the opposite end of the shelf from the zitelli's. Not sure who this book is for. Hospitalists use nelsons and online stuff. Not for the ICU or ER. Maybe if this was pocket book or on PDA it would be more useful. Not useful as a reference text.

I know what the stated goals of the book were and that it's hard to write a text in a rapidly evolving field, but i was still disappointed in this book. This is definately a niche that needs a good text, perhaps one strewn with pearls and algorithms.

Although I am a paediatric cardiologist, I have never really understood paediatric heart sounds. I was born into the subject in 1980 with an ultrasound machine attached to my right wrist and could not wait to dispense with the guesswork that auscultation seemed to entail. Now I wear my stethoscope only for decoration or to add a touch of old fashioned authenticity for suspicious parents. The title of this publication seems quaint for the 1990s, at a time when technology has taken over from the stethoscope and a child with a complex heart malformation can be evaluated and surgically treated without the bell ever touching the chest.

The reasoning behind children's heart sounds and murmurs is laid out clearly and Lehrer is obviously a skilled, thoughtful, and experienced auscultator. I recommend especially the chapter on systolic murmurs to many of my consultant paediatric colleagues who remain unable to distinguish a classic innocent murmur from that of a ventricular septal defect. Many children arrive in our clinics having been referred for unnecessary echocardiograms. On several occasions I became concerned that the author had lost touch with modem terminology; endocardial cushion defects are not commonly described as such nowadays, being referred to as atrioventricular septal defects. Also, to talk of surgical pulmonary valvotomy, when the established treatment for pulmonary stenosis for nearly ten years has been a balloon valvuloplasty, seems strange. There is a brief, if slightly naive, overview of some major heart malformations, but this is perfectly suitable for undergraduates.

The tape provides good examples of the different types of heart sounds and murmurs found in children. I was surprised that one of the most common, that of pulmonary stenosis, was not included. However, familiarity with the limited nature of possible diagnoses should dispel the panic often felt by undergraduates and even, in some instances, postgraduates when asked to auscultate a child's heart.

One of the joys of pediatric cardiology is the formulation of a clinical impression based only on the history and cardiovascular examination, perhaps supplemented by an electrocardiogram and a chest x-ray. For cost effectiveness and efficient use of resources, screening and referrals must be based on these cornerstones of clinical assessment. It is therefore timely and appropriate to reiterate the importance of bedside clinical evaluation. Lehrer's book emphasizes that auscultation is a diagnostic skill that still has great influence in the investigation of the pediatric patient thought to have heart disease.

Understanding Pediatric Heart Sounds is a well written, succinct book combined with a 20 minute audiotape. It contains 15 chapters, the first 3 of which concern anatomy and physiology, physical principles of auscultation, and an overview of the history and physical examination in pediatric cardiology. A brief chapter explains the methods of phonocardiography and external pulse recordings. The remaining chapters extensively detail the auscultation of normal heart sounds, abnormal cardiac sounds, and murmurs. The differential diagnosis of murmurs and the murmurs characteristic of both common and complex entities are presented. The relatively brief audiotape includes normal and abnormal heart sounds, as well as examples of murmurs common in young patients. For maximal clarity, the use of a stethoscope placed near the speaker is recommended when listening to the tape.

The major strengths of this book are its crisp, clear cut, and readable style and its thorough descriptions of auscultatory findings and of the mechanisms of cardiac sounds and murmurs. The integration of the text with ample, high quality illustrations allows the reader to proceed at a brisk pace. The reference list includes many classic works, and the index is comprehensive. On the audiotape, the reproductions of heart sounds are excellent. On the other hand, many of the murmurs are of only fair quality and are not always representative of clinical findings in young patients. However, having tried many times to simulate or present common murmurs in an audio format, I have great empathy for authors who try to reproduce them accurately. We live in an age of incredible electronic sophistication, but our ability to record or produce heart murmurs has lagged well behind our skill at other endeavors. Learning the art of auscultation requires accurate information, tutelage, and repetition. The reader-listener might be better served by paying more attention to the easy to follow text and the well produced heart sounds and less to the simulated heart murmurs.

Although the style is consistent, chapter 5 may be somewhat confusing to both novice examiners and experienced practitioners. It presents a revision of traditional areas of auscultation. Instead of referring to the well known mitral and tricuspid areas, the author uses a format that includes left and right ventricular and left and right atrial areas, as well as the traditional aortic and pulmonary areas. Although it has some potential benefit for enhancing communication, this format has the drawback of substantial overlap between these regions. Furthermore, the author states subsequently that the cardiac structures in congenital heart disease may be "displaced from their usual locations." Instead of a concentrated focus on specified areas of auscultation, a description of murmurs in relation to well-known landmarks, such as the mid left sternal border, the lateral second left interspace, and the suprasternal area, might be more effective. The technique of inching, moving the stethoscope in small increments over the entire anterior chest and also over the back, is described briefly, but it deserves emphasis. Also, the technique of dissection, which involves concentrating on one portion of the cardiac cycle or one heart sound to the exclusion of others, is often beneficial but is not discussed in the text.

A few minor omissions include the low pitch of innocent carotid bruits, the left lateral displacement of the aortic closure sound in congenitally corrected transposition of the great vessels, and the diffuse nature of continuous murmurs in patients with tetralogy of Fallot with pulmonary atresia and pulmonary blood flow derived from aortopulmonary collateral vessels. The narrow split of the second sound in patients with pulmonary hypertension is shown in a figure but is not mentioned in the text. In addition, the auscultatory variations of murmurs due to ventricular septal defects should have received more extensive review.

Despite these minor criticisms, I found Lehrer's work refreshing, concise, and a pleasure to read. The author's clear style makes the book, and at least a good portion of the audiotape, eminently suitable for students, house staff, and practitioners who wish to enhance their listening skills for pediatric cardiovascular examination. I wholeheartedly agree with the author that auscultation should not become a lost art.

This 230 page soft cover book, by Steven Lehrer, MD, is a comprehensive review of the literature related to heart sounds. The author writes simply and clearly. The book is almost an abbreviated text of pediatric cardiology and could be titled "Pediatric Cardiovascular Assessment and Diagnosis." Much of the material has been previously covered in such books as Pediatric Cardiology for Practitioners, by M. K. Park, and two books by J. K. Perloff, The Clinical Recognition of Congenital Heart Disease and Physical Examination of the Heart and Circulation.

The figures and tables are almost exclusively reproduced with acknowledgment from previous publications. Although little new information is included, the manner in which the author presents the material is outstanding and goes much beyond the understanding of pediatric heart sounds. The references at the end of each chapter are extensive and excellent for the student who desires to pursue the subjects in great detail.

The chapter on heart murmurs will be particularly helpful for a better understanding of the origins of such sounds. As pointed out by the author, functional benign heart murmurs are common in children, yet perplexing for the practitioner to identify. Most care givers feel uncomfortable with any murmur that is grade 3 in intensity and will refer the child to a pediatric cardiologist for assessment.

The glossary is very beneficial, as is the chapter "Transcript for the Supplemental Tape." The tape should be most useful to the sincere student of auscultation. With the faster heart rates of younger children, the use of one's stethoscope under the tutored wisdom of an experienced pediatric cardiologist is indispensable!

The first three chapters of this book will be most helpful to all medical students. Family practice and pediatric residents and pediatric cardiology fellows will find the book full of good information in summary form. For the practicing care giver, the book will be an excellent reference text for refresher information.

ISBN 0 7216 2387 5. Although I am a paediatric cardiologist, I have never really understood paediatric heart sounds. I was born into the subject in 1980 with an ultrasound machine attached to my right wrist and could not wait to dispense with the guesswork that auscultation seemed to entail. Now I wear my stethoscope only for decoration or to add a touch of old fashioned authenticity for suspicious parents. The title of this publication seems quaint for the 1990s, at a time when technology has taken over from the stethoscope and a child with a complex heart malformation can be evaluated and surgically treated without the bell ever touching the chest.

Bereft of his portable echo machine, Lehrer has put together a package that is ideal for both undergraduates and postgraduates in paediatric training. This is not a very useful study guide. It's definitely not written for leading a group. Most questions are along the lines of "what did Solomon really mean here?" Eccesiastes doesn't lend itself to inductive study. You need to be creative around themes that are brought out in the chapter. IV's Lifeguide study is much better.
